Greater Houston Ophthalmologist Bernard Milstein, M.D. relayed information on nearsighted risk in children. “According to researchers reporting on the risk of nearsightedness or myopia in children, increased time spent in natural light and viewing distant objects during childhood may reduce the risk of nearsightedness,” reported Dr. Milstein of The Eye Clinic of Texas. A review of data of 10,400 participants showed that for each additional hour outdoors per week, the chance of developing myopia dropped by about 2%, according to a presentation at the annual meeting of the 2011 Annual Meeting of the American Academy of Ophthalmology. It was reported that myopic or nearsighted children included in the study tended to spend about 3.7 fewer hours per week outdoors than children with normal vision or hyperopia. Furthermore, increasing outdoor time might reduce the chances of myopia worsening, according to the release.

For many people, cataract surgery can be a liberating experience – they have a chance to throw away their glasses. And this is true for many people.

Just about everyone over the age of 40 needs to wear glasses. They either need glasses for distance or for near. With the advent of intraocular lenses, and the improvements in the technology, combined with LASIK surgery, it is now no longer necessary to wear glasses. Particularly, if you are having cataract surgery, there are multiple options available for patients to choose from.

The ReSTOR lens, a multifocal lens, can provide both near and distance vision and LASIK surgery can cure any remaining astigmatism.

Some patients prefer a monofocal lens. The options here are lenses that correct for astigmatism or correcting the astigmatism after surgery with LASIK. Both are excellent alternatives.

Monovision, one eye corrected for distance and one eye corrected for near is an excellent choice for patients who are sure they can adjust to this type of vision. In patients just having LASIK, we always give them a simulation of the monovision using contact lenses. It is more difficult to simulate in people who already have cataracts because their vision is restricted.

Economics of Medical Technology-Dr. Milstein

There are new technologies being developed in all fields of medicine and ophthalmology is no exception. Ophthalmologists have the ability to do incredible things to improve people’s vision and their lives. The future of medicine and ophthalmology is even more impressive. There is, however, an obstacle, and that obstacle is money. The economics of healthcare and the introduction of new technology inevitably increases cost.

Our country is facing a real dilemma. On one hand we have the ability to provide advanced technology to cure disease and extend the quality of life and on the other hand, we have millions of people who have no medical insurance at all and cannot afford even the basics of medical care. The ethics and morality of this problem have challenged and eluded all of us.

We must develop a system that provides basic healthcare for those who cannot provide it for themselves, but we also must have the ability to encourage the development of new technology and the opportunity to fund that development. That means that physicians should be able to charge a fee that is not controlled by the government and individuals should be allowed to purchase services over and above basic “government issue,” healthcare.

I recently returned from the annual meeting of The American Academy of Ophthalmology. As always, the meeting is a showcase of new and interesting therapies and technologies. There is a real interest in the technologies to improve and expand surgery of the lens. We are no longer focused simply on removing a cataract. We are now expanding our ability to use the lens, and lens surgery, to improve our lives.

An instrument is being developed that can examine the lens and predict and diagnose diabetes far earlier than current blood tests. Another technology is a method that would allow patients to measure their own intraocular pressures at home.

In addition to intraocular lenses that correct myopia, hyperopia, astigmatism, and presbyopia, there are lenses being developed that will allow for them to be focused once they are placed into the eye and even lenses that will focus themselves. Intraocular lens technology will continue to improve and expand.

As intraocular lens technology improves, the systems to remove and replace the human lens will continue to be advanced. Improved visualization for surgeons continues to be developed and the use of lasers to assist during the surgical procedure are being perfected.

All of these new technologies will continue to improve cataract and refractive lens surgery. Many of these technologies already exist and we are able to employ them today. These are exciting times and there is an exciting future for ophthalmologists and our patients.
